Governance and Licensure of the Clayberg:

The Clayberg is a 49-bed Intermediate Care Facility located at 625 E. Monroe Street Cuba, IL. The Clayberg is licensed to provide Intermediate Care and Skilled Care Services through the Illinois Department of Public Health. The Clayberg is also certified as a Medicare Provider of care and services by the CMS (Centers for Medicare and Medicaid Services).  The Clayberg is owned and operated by the Fulton County of Illinois and is governed by the Fulton County Board. Immediate day to day operations of the Clayberg are under the direction of Administrator Martha Danielson, Assistant Administrator Kevin Stoneking, and Director of Nursing Debby Powell MSN The Health Committee of the Fulton County Board meets monthly with the Administrative staff of the Clayberg and has immediate oversight of the Clayberg operations. The Clayberg employs 65 professional and non-professional staff in the provision of 24 hours a day, 7 days a week care and services to the elders who reside here.



Care and Services provided at the Clayberg:

The Clayberg staff pride themselves in giving personalized, individualized care and services to those who reside here. The staff are experienced and knowledgeable and take into account the elders personal requests and desires as well as medical, physical, psychological, or other needs the elder may have. The Clayberg is staffed with qualified, caring, and experienced staff:

  • Nursing Staff is comprised of Registered Nurse (RN) each day, Licensed Practical Nurses (LPN) and Certified Nursing Assistants (C.N.A). The nursing staff and the nursing care and services provided for the elders residing at the Clayberg are under the direct supervision of the Director of Nursing. Each elders care is ordered by his/her attending physician.
  •  Physical & Rehabilitation Therapy Services: The RehabCare Rehabilitation Therapy Inc. has joined the Clayberg staff in providing professional therapy services to those who reside at the Clayberg, with physician order. The Clayberg employs a Registered Nurse with Restorative Nursing Certification to facilitate the processes of planning and implementing restorative programs specific for each elder as appropriate.
  •  Dietary & Food Services: The Clayberg utilizes the services of a consulting Dietitian on a monthly basis per physician order and in compliance with appropriate state and federal regulations.
  •  Housekeeping & Laundry Services: The Clayberg staff provide laundry and cleaning services during the first and second shifts. Personal laundry services are also provided as needed and desired by the elders.
  •  Activities & Socialization: The Clayberg employs a fulltime Activity Director and assistants to provide a variety of activities and social opportunities for those who reside at the Clayberg. Activities are planned during both daytime and evening shifts to meet the needs of those residing at the Clayberg. Special interest groups, music, entertainment and more are available throughout the month. Holidays are celebrated as are birthdays and other special event days throughout the year. Family and significant others are included and encouraged to participate in those events or activities that bring enjoyment to them and their loved ones. Volunteers, staff and families participate in providing special events throughout the year.
  •  Admissions & Social Services: Full time Social Services staff are available to meet with prospective clients of the Clayberg or with current elders and families of the Clayberg. Social Services play an important role in the elder s’ transition into long term care setting as well as facilitating day to day adjustments and/or problem solving.


Facility and Grounds: The Clayberg is located in the southwest corner of Cuba at the end of Monroe Street. It a quiet, rural-like setting with beautiful lawn, trees, and flower gardens. The west windows overlook a large empty, well-groomed lot where elders are often able to observe deer and other wild life.

  • Maintenance staff are available to provide preventive and responsive maintenance of the facility and equipment. Effort is made to maintain a workable and pleasant environment for those living and working at the Clayberg. Maintenance staff have experience and current certifications needed to maintain a safe and operational facility.  
  • Volunteers and staff have invested time, energy, and resources in the exterior property and grounds to provide a pleasant garden, patio and walking path for residents and families to enjoy. Activities and events are scheduled in the garden or on the patio to provide opportunities to enjoy the outdoors.

Transportation: The Clayberg owns a 12 passenger van which accommodates 2 wheelchairs. The van is utilized to provide short trips away from the facility for elders who have an appointment for medical or other purposes and/or for outings such as: going out to eat, sightseeing around the county, or to a local concert or ball game.
