The Clayberg is committed to providing services, information, education and recreation for our residents. Our skilled, trained and professional staff recognizes the wisdom and experience of our residents and demonstrates an awareness and respect for the aging process.
Upon the initial inquiry, our team will seek medical and social information regarding the potential resident. Our Admission Coordinator will establish a pre-screening meeting to discuss this process in further details and arrange a tour of our facility.
This is an opportunity for our Admission Coordinator to met the potential resident and family members in their current living setting. During this meeting, our team members will identify and barriers that may need addressed, focus on the needs of the potential resident and set goals for the admission.
Our team members will review current medical treatment, the physician referral process, payment options and arrange for the tour of the facility, if needed.
We have established three different room rates based on the level of care required.
The Director of Nursing will complete and assessment during the Pre-Screening process that will determine this level of care. The assessment will the be completed at the least every 90 days to ensure the resident receives the accurate level of care.
